For Providers: How to Refer a Patient for SPRAVATO® Treatment
Step 1:
Complete the Provider Referral Form and gather copies (front & back) of the patient’s pharmacy and medical insurance cards.
Step 2:
Upload the completed referral form and documents through our secure portal.
Step 3:
You're done! Our team will take it from here and follow up as needed.
